Ultrasound (US) has demonstrated to play an important role in the diagnosis and follow up of patients with musculoskeletal diseases but its applications in the rheumatological field are rapidly expanding also beyond the strict use for the assessment of joint and periarticular disorders, moving to the assessment of extra-articular involvement (for example salivary gland damage) and vascular inflammation.

This imaging tool can be used directly by rheumatologists during patient evaluations, as a real-time “bedside” approach, thereby enhancing diagnostic accuracy and reducing the time between diagnosis and the initiation of appropriate treatment, that is crucial for most of the rheumatological diseases.

However, the implementation of US in daily clinical practice is still limited mainly due to the lack of trained personnel. Moreover, due to physical and technical limitations US cannot provide adequate information on every clinical question. Integrated imaging (MRI, CT and conventional radiography) should be chosen in those cases in order to add to the clinical puzzle the missing
pieces.

This course is designed to deliver a cutting-edge update on the applications of US and integrated imaging in Rheumatology, targeting both rheumatologists with ultrasound expertise and those without, providing pragmatic guidance on how to optimize the use of ultrasound and imaging for the clinical evaluation of rheumatologic patients. It encompasses a comprehensive examination of both inflammatory and non-inflammatory rheumatologic conditions and includes practical imaging sessions, that should increase confidence for the use of US (and integrated imaging) in daily clinical practice and for research purposes.

Sonoanatomy 2024 took place this past week in Barcelona. Sonoanatomy is an interactive and multidisciplinary course organized by Instituo Poal de Reumatología at the University of Barcelona under the umbrella of Eular and Euroson. It showcases advances in the understanding of sonoanatomy.

¿Habéis hecho alguna vez el comentario “va a llover porque me duelen mucho los huesos”? Un estudio realizado hace algún tiempo por el Instituto Poal de Reumatología de Barcelona y la Unidad de Salud Articular de Bioibérica Farma quiso comprobar si este dolor que notan los pacientes cuando cambia el tiempo era real. Y descubrieron que sí, que el paciente tenía razón y que los cambios de temperatura y de presión atmosférica afectan a las personas con artrosis y artritis.

Concretamente, se vio que unos días antes de que llueva (cuando la presión barométrica baja) los pacientes artrósicos sienten más dolor en sus articulaciones. Sin embargo, las personas que sufren artritis reumatoide perciben un mayor dolor articular cuando bajan las temperaturas.

La doctora Ingrid Möller, reumatóloga y directora del Instituto Poal de Barcelona, explica que hay un 40% de pacientes que son susceptibles de predecir los fenómenos meteorológicos según el dolor de sus articulaciones. “Por suerte, el cambio de tiempo sólo influye en la percepción del dolor y no agrava la enfermedad”, nos explica Möller.

Tratamientos recomendados

La especialista aconseja a los pacientes que en invierno, cuando bajan más las temperaturas, apliquen calor en la zona del dolor y realicen ejercicios suaves como caminar o nadar para desentumecer las articulaciones.

Aparte de los analgésicos y antiinflamatorios para calmar el dolor en las articulaciones, Möller recuerda que existen medicamentos específicos para la artrosis como los condroprotectores, que contribuyen a mejorar el dolor y la capacidad funcional del enfermo con un alto perfil de seguridad.

El estudio se  elaboró durante dos años con pacientes del Instituto Poal y contó con la colaboración del Servei Meteorològic de Catalunya. En él participaron 92 personas con afectaciones reumáticas y otro grupo de 42 personas sanas para comparar sus percepciones y reacciones a los cambios meteorológicos. El estudio se publicó en Proceedings of the Western Pharmacology Society.
